How Do I Apply for the Novo Nordisk PAP?

Applying for the Novo Nordisk PAP is fairly straightforward, though you’ll need some help from your healthcare provider. Let’s walk through the steps to make it easier.

Preparing Your Application

Start by gathering all the required information:

  • Your healthcare provider’s name, state, and email address. You must verify your healthcare provider’s email address before starting the application.
  • If you have Medicare, your insurance card(s).
  • Tax documents from the most recent year you filed taxes.
  • You may need a copy of your two most current paycheck stubs, your last year’s federal income tax return, Social Security income statements, or other proof of income.

Submitting the Application

You can apply online or download the application. The online application is only available in English, but paper applications are in both English and Spanish. You will not be able to complete the application without the assistance from your healthcare provider.

  • Online: Your healthcare provider can complete their sections online. Once they’re done, you’ll receive a code by email to complete your part.
  • Paper: Download the application, fill in your sections, and bring it—along with your proof of income—to your healthcare provider. They’ll need to complete the Prescriber and Rx sections, sign, and either fax or mail it directly to Novo Nordisk.

After you submit the application, Novo Nordisk will process it within 10 business days. Both you and your healthcare provider will receive a decision by mail, with an option for an automated phone notification if you opt-in.

If you’re approved, you’ll be officially enrolled.

  • If you’re uninsured, enrollment lasts for 12 months. If you have Medicare, you’ll be enrolled until December 31st, so make sure to apply by November 30th each year for continuous coverage.
  • When your enrollment period ends, reapply if you want to keep receiving assistance. Medicare Part D patients can reapply for the next year’s program starting October 15th.

How Will I Get My Medication?

Once approved, Novo Nordisk will send your medication directly to your healthcare provider’s office within 10–14 business days. You’ll pick it up there, as the program doesn’t allow for home delivery.

For convenience, most medications come with automatic refills, which are managed by your prescriber. Just note that some exclusions apply, such as NovoPen Echo®, Zegalogue®, and certain Ozempic® and Rybelsus® prescriptions. Your provider can opt-out of automatic refills if needed.
