If your insurance covers Mounjaro, the card reduces costs to as little as $25 per month, with an annual savings limit of $1,950. If your insurance does not cover Mounjaro, the card offers up to $473 in savings per prescription, capped at $3,311 annually. The card expires on December 31, 2025, for those with insurance coverage. You must activate the card and meet all requirements every time you use it.

You must also have Medicare insurance that covers the prescribed medication or product, which must be listed on PAN’s covered medications. Additionally, household income must fall at or below 400% of the Federal Poverty Level.
The HealthWell Foundation
The HealthWell Foundation is a nonprofit that helps underinsured individuals with their healthcare costs.
